The Arizona check cards

Check the person. Check the pharmacy.

Collect full names before paying. Ask for the person’s job title and licence number. For someone licensed in another state, ask for their Arizona remote-care record number.

Different names need different checks

  1. Care professional Who assesses you?
  2. Pharmacy Who supplies it?
  3. Product What is being supplied?
Keep all of the names together with the written quote.

A licensing board is a state office responsible for checking licences for a type of health worker.

Check the person and the pharmacy on their own.

Check the person giving care

MD is short for medical doctor.

Arizona’s Medical Board search shows MD licence details.

Public written decisions by the board about a doctor’s work also appear there; they are called board orders.

An osteopathic doctor, another kind of medical doctor, uses the letters DO.

Arizona lists DOs through a separate board.

For a nurse trained to do extra care tasks, check the nursing licence and the record for that extra work.

That extra record is called an advanced-practice certificate. Use the Nursing Board’s links to check it.

Nurses with nurse-practitioner training can assess your health and decide what care is needed.

On October 3, 2026, Arizona rules required Nursing Board permission for a nurse practitioner to prescribe.

Their prescribing had to stay within their training.

Ask the nurse for the full name of that certificate. Ask the Nursing Board: “Do these records allow this nurse to prescribe the medicine?”

PA means physician assistant, a licensed type of health worker.

Arizona has a PA board with its own licence search.

PA prescribing depended on training and ability under Arizona rules checked October 3, 2026.

The PA board checked how many hours a PA had worked in medical care.

Those hours set whether the PA needed a doctor’s written agreement to oversee their work.

Ask the PA board whether the person may assess you and prescribe the medicine, and whether a doctor must oversee their work.

Match the full name and job title. Read the board’s written decisions and ask it to explain anything unclear.

Check the pharmacy

As of October 3, 2026, Arizona used pharmacy permits and separate pharmacist licences.

A pharmacist’s personal licence is different from the permit for the pharmacy.

As of October 3, 2026, a pharmacy sending medicine into Arizona from another state also needed an Arizona permit.

State pharmacy licence searches can be reached from the public lookup page of the Food and Drug Administration (FDA).

Ask for the actual pharmacy name and its Arizona permit details, even when the care service has another name.

Match the named person and the named pharmacy separately. Then ask about the exact medicine.

A blank board listing has limits.

On October 6, 2026, Arizona law limited what a licence board could show on its website.

This included decisions about problems with a person’s work, such as penalties or limits on their practice.

Ask the board to provide additional records that are public.

A blank online record does not prove the person has never faced a complaint or a penalty.

Keep the names and any records you find. Ask the board about information you cannot resolve.
